A 16-year-old adolescent with severe flat foot deformity ( flexible) with no sign of coalition found. The pain was over medial talar region and recalcitrant to conservative treatment. He was treated by multiple osteotomies and lateral column lengthening. 
Following steps were involved during the treatment:

Step 1: Fibula graft harvesting,
step 2: Medial sliding calcaneal osteotomy,
step 3: Lateral column lengthening and Evan's osteotomy.
Step 4: Cotton osteotomy (dorsal open wedge medial cuneiform
