But peroneal tendons were started subluxating on walking on an uneven surface. For more than 5 years, patient walked with peroneal subluxation and developed a lot of pain on regular walking due to peroneal tenosynovitis and friction tears. MRI has confirmed the diagnosis and showed that superior peroneal retinaculum was torn and elongated causing lateral subluxation of underlying tendons. Also, it was found that the retromalleolar groove was shallow, couldn't accommodate tendons sufficiently. Tendons were exposed, found lax SPR and very shallow peroneal groove.
Lots of longitudinal tears were found on peroneal brevis and repaired with intratendinous debridement and suturing
Finally, peroneals were inserted back to its deepened groove and SPR was repaired
