A 45-year-old male had chronic pain in left ankle - history of a non-resolving ankle sprain.
He has twisted his ankle 5 to 6 times in the last 2 years.
 
Treated for TB, Rheumatoid and septic arthritis in many hospitals in another state. Had recalcitrant pain. 
I found lateral ankle ligamentous instability with resultant 15 to 20 degree ankle varus deformity. 
MRI was showing limited ankle arthritis. 
Clinically ankle had a good range of motion.
Treated by supra malleolar open wedge osteotomy with lateral ankle ligament Reconstruction.
