Presenting an interesting case here:
He is 60-year-old active men who met with Road traffic accident 6 months back. He had gross painful swelling in his injured leg and was not able to stand on it. He went to an orthopod, an x-ray was done and the patient was advised that there is minor ligamentous ankle sprain which will be ok with brief rest and some drugs. For continued pain for a long time, he visited multiple clinics.
I got this patient recently and I found severe fracture dislocation of the talus with subluxation of all 3 joints- talonavicular, subtalar and tibiotalar. (known as Hawkins type 4 injury)
He was walking on a missed injury for almost 5 months and had developed cavovarus foot ( walking on the lateral border of foot).
Open Reduction was almost impossible and the prognosis is bad after ORIF.
So, with the aim to get the stable and pain-free plantigrade foot and to spare his ankle joint biomechanics, double joint fused has been done.
